Home Health

Care in the Comfort of Home

Experience personalized, high-quality healthcare without leaving home. Our clinician-led home health services help patients recover, manage conditions, and maintain independence where they feel most comfortable—at home.

Home Health vs. Home Care

While Home Care focuses on non-clinical support such as personal care, companionship, and help with daily activities like bathing, meal preparation, and housekeeping, Home Health goes a step further. Home Health is skilled medical care: clinical services including nursing, various therapies, and help managing medications, all under the direction of your doctor. It is typically prescribed by a physician after surgery, injury, or a change in medical condition.

Many patients benefit from combining both services for complete support at home: medical care when needed, and daily living assistance to promote comfort and independence.

Our Process

Your health and well-being are at the center of everything we do. From the moment you reach out, our team listens to your goals, needs, and preferences. We work closely with you, your family, and your healthcare provider to create a customized care plan designed to promote healing and quality of life.

When your care begins, a member of our team will meet with you and your caregiver (if applicable). This starts with a phone call from the agency to schedule your first appointment. During your initial visit, we will review your plan of care and prioritize safety, offering tips to help you move around your home and reduce your risk of falling.

How often will we visit?

The number of visits you receive will depend on several important factors, including your doctor’s orders, what our providers observe during your ongoing assessments, and what your insurance plan covers. Some patients may have 1-2 visits each week, while others may only need one visit per month. Your care plan is personalized to your needs and may change over time based on how you’re doing.

How is it paid?

Home health services can be covered by:

  • Medicare

  • Medicaid

  • Managed care plans

  • Tricare/VA

  • Private insurance plans

Our team will help you review your benefits, explain coverage, and help coordinate authorization if needed.
